Your First Visit
When you first arrive we will go over your medical and dental history information. For your convenience we have placed our forms here on the web site for you to download and fill out before your appointment. We will also verify your dental insurance so you are able to make informed decisions about any future dental treatments. During your visit, we will take photos that are used for diagnostic purposes as well as documenting treatment results. We will then do a clinical exam, take x-rays if needed and perform either a general or deep cleaning as indicated by the exam. After the exam, Dr. Evelyn will explain any findings with you and outline a treatment plan if needed. Dr. Evelyn likes her patients to be as well informed so they are able to make informed decisions about their dental care and treatment.
Comfort Care
When designing the office, Dr. Evelyn chose colors and designs that were soothing and relaxing for patients. We also have flat screen TV’s, bottled water, music and blankets to make your visit as comfortable as possible.
Financial Arrangements
Our financial policy allows us to provide you with the best treatment available and does not limit your health to just what your plan covers. We will be happy to submit your insurance paperwork for you at the time of treatment. If you have any questions about your insurance, please feel free to call for more information. We offer several payment options for your convenience. We accept cash, as well as all of the major credit cards. We also offer Care Credit and Capital One financing programs.
